Aims
To determine whether cytomegalovirus (CMV) Infections represent a risk factor for acute rejection, graft failure and long-term complications in patients receiving a first cadaveric renal transplant.
Interventions
Daclizumab versus thymoglobulin. All patients were given cyclosporine, mycophenolate mofetil (MMF) and steroids.
Participants
109 low-immunologic-risk patients receiving a first cadaveric renal transplant.
Outcomes
Renal function measured as estimated creatinine clearance, patient and graft survival, biopsy-proven acute rejection, incidence of cancers and lymphoproliferative disorders, occurrence of new-onset diabetes, incidence of infections excluding CMV infections episodes and cardiovascular complications.

CET Conclusions
In this multicentre trial (ECTAZ) comparing two doses of daclizumab with thymoglobulin for induction, plus cyclosporine, MMF and steroids in recipients of cadaver kidneys, the effect of CMV infection and its influence on outcome was examined in comparison with patients who had no evidence of CMV infection. This suggested that CMV infections are associated with a higher incidence of other types of infections but there is no difference in other long term complications. The authors suggest that anti CMV prophylaxis could decrease the risk for long term related CMV complications.
