Mobile health medication adherence and blood pressure control in renal transplant recipients: A proof-of-concept randomized controlled trial.

Aims
To assess the feasibility, acceptability and preliminary outcomes of a prototype mobile health (mhealth) medication and blood pressure self management system for kidney transplant patients with uncontrolled hypertension.

Interventions
Participants were randomised to a standard care or mhealth group. The standard care group received care at the MUSC kidney transplant clinic, which included visits every 4-6 weeks, education on post-transplant medical care and 24 hour phone availability of transplant co-ordinators. The participants continued to use their medication tray with its reminder function disabled. The second group of participants used the mhealth system, where the medication tray if unopened would chime for 30 minutes, if the compartment remained unopened after the initial 30 minutes a phone call or text message would

Participants
20 hypertensive kidney transplant patients identified as non-adherent to their medication regime.

Outcomes
The outcomes included medication adherence and blood pressure measurements.

Follow-up
3 months.

CET Conclusions
An integrated mobile health alert system incorporating digital dossette boxes, mobile telephones and sphygmomanometers was tested in improving patient compliance and self-monitoring. The patient population was previously identified as non-adherent during a screening period; however a quarter of patients approached to take part declined to do so. Only a small number of patients were finally randomised, however the study group demonstrated higher compliance rates. The study group also had reduced measured blood pressure compared to the control group at monthly clinic reviews.
